The Concordia Health team are passionate about working closely with community and voluntary organisations. By focusing on partnering with local groups who provide support for specific residents we are able to engage and provide healthcare services to some of the typically hard to reach people.
Our community engagement activities involve ongoing outreach work with 3rd sector organisations, which incorporate health checks, group discussions on relevant health topics, one-to-one sessions and health drop ins. In partnership with the local PCTs, health authorities, and various public and private sector organisations we become actively involved in community events such as festivals and health and well being days, proactively manage events and workshops bring complimentary support groups together and arrange health promotion campaigns with community leaders.
With the voluntary sector suffering with funding cutbacks it is even more important that local residents know of their services and utilise them. Using Concordia as the mechanism to help bring these groups and residents together has been recognised by local MPs and the National Health Inequalities Support Team of the Department of Health. Our GP Led health centre in Mitcham who has been leading the community development plan has now been shortlisted for the for three awards at the 'General Practice of the Year Awards 2011' in recognition for its achievements over the last year in this area.
