Services

We tailor our services to local requirements. We have a proven track record in the delivery of traditional and more innovative models of primary care and urgent care. In particular, we have had major impacts on reducing health inequalities, and in reducing secondary care costs, in the localities in which we work. We work with local PBC consortia to develop local NHS communities and to introduce new patient pathways.

GP Practices

We currently run five general practices across London. These include a mixture of nurse- and doctor-led practices with walk-in services and extended opening hours.

The Wilson Health Centre, Mitcham
Child and Family Centre, Barking
Porters Avenue Doctors Surgery, Dagenham
Parkside Medical Centre, Camberwell
Melbourne Grove Medical Practice, East Dulwich

Urgent Care

We work in partnership with GP practices, A&E departments and out-of-hours providers to ensure that all the patients we see get the right healthcare and treatment as soon as possible. We currently provide urgent care services in St Helier Hospital and St George's Hospital A&E Departments, and we have been able to redirect 65% of the patients we see.

Secondary Care Savings

We have developed referral management and GPSI services to enable local consortia to have greater control of patient pathways and thus enable considerable cost savings.

We have also developed more effective patient pathways within hospitals to support clinicians by ensuring patients are moved through the departments and discharged as soon as clinically appropriate.

Healthy Living

Combining our experience in healthy living care and advice with that of the local NHS Trust's public health team, partnering with leisure facilities and health trainers, we offer an integrated lifestyle service - making healthy living easier and a part of everyday life.

Health Education

Our Making Sense of Health programme is a modular course that teaches children at both primary and secondary schools how to use the NHS and how to lead a healthy lifestyle. This includes advice on healthy eating, smoking, drug misuse and sexual health. The course has been tried and tested through 300 schools across the country and focuses on teaching the teachers to run the programmes.
