We work in partnership with GP practices, A&E departments and out-of-hours providers to ensure that all the patients we see get the right healthcare and treatment, by the right clinician at the most appropriate time and in the best environment. We have experience in providing urgent care services in St Helier Hospital and St George's Hospital A&E Departments where we directed approximately 65% of patients we assessed to a more appropriate service, and more recently we are working in Lewisham University Hospital A&E department with a Joint Management Board to improve performance and reduce inappropriate admissions and referrals. Our Walk in Centres have performed well above expectation and we continue to provide high quality, flexible and responsive services to challenging and deprived areas of the country.
The success of our integrated A&E model has been used as a case study by the PAGB and the typically low cost set up for the service and the resulting streaming of patients away from high tariff areas, has resulted in considerable savings by the NHS Trusts, as well as high patient satisfaction. Our model of assessing, signposting and treating A&E attendees has also be recognised as best practice by the South West London Acute Commissioning Unit.
Our team also has experience of creating single points of access and integrated urgent care models in isolation or within consortia and we would like to work with partners as part of the 3-digit number programme, to support the creation of the CMS DoS and develop local patient pathways.
